MADISON BOOTH HEALTH INFORMATION

Each group of artists must apply for a Temporary Body Art Establishment License. This only needs to be submitted and paid for once by one person from the booth and will cover all of the artists that are in your booth. Please notate in the form all the artists that are in your booth for which the license will cover.
