DAVENPORT ARTIST HEALTH INFORMATION

Every artist tattooing at Tattoo Fest Davenport must fill out this form and provide the requested information. Failure to submit this information on time will result in not being approved to tattoo. You must provide the government name and Iowa Tattoo Permit for each artist in the booth. (This number is found on their Iowa Tattoo Permit)
